4) Health and Safety Acknowledgement
All three certifications are required. If you don't have one or all, you can schedule to get trained. In order to move to the next step, it is required that you get certified in each. We will reimburse you for the training once you complete 40 hours of service.
You can schedule training on any of the following websites if you do not have these certifications.

6) DCW Parents of Adult DDD Member Information

7) Authorization Requests
Accepted documents include US Passport, Driver's License, Birth Certificate, or Social Security Card.
8) Arizona Centralized Background Check (CBC)
Download CBC Quick Set Up Instructions Here
Your Background Check will be submitted directly through the CBC portal.
9) Proof of Valid Transportation & Insurance
Each of the following is required:
ADOT Motor Vehicle Report (you can get it here)
Copy of your vehicle's current registration
Proof of car insurance (a copy)
10) References
You must submit three references and include their name, relationship, number of years you've known them, and an email address and phone number, or click here to download a reference request form.
